Use the blank form above if you haven't received a form via email from the club. New to the club assistant coaches will need to fill this out. Attention trainers:

The soccer fields at Seapark, DeSoto, the the Satellite Beach Library Sports Complex are open only to individuals and groups registered with SBSC or the Satellite Beach Recreation Department (contact Kerry Stoms). The city does not permit individual trainers to use city fields without the authorization of either the city or SBSC and the payment of all applicable fees. If you are not registered with SBSC and train players on SBSC fields, you will be asked to leave and may be charged a substantial fee. To register, please download, fill out and submit the Affiliated Trainer Application along with the appropriate fee. All field usage must be coordinated with SBSC. If you have questions, please contact SBSC.
